Use this option to add a family member to a current membership. A form will be completed separately during the checkout process to identify the current member. The form has name field for one family member. Please add only one family at a time using this form. Please note - a $4 “convenience fee is included to cover the web page and credit card fees. You may download the form on the Contact Us page and pay via cash or check ($30 per additional family member) to avoid this fee.
Any two or more members of the same household related by blood, marriage, or common law marriage. The board can approve any other “family” arrangements not listed here, so long as that rule is applied equally to everyone.
